Does it mean those who are unable to meet the cut-off points pay fee?  while those who meet the cut-off points do not pay fees at all.

it means that those who meet the cut off points pay fees which has been subsidized by the government.
The government pays some part of the fees for those who meet the cut-off points whiles those who do not meet the cut-off points pay everything themselves without government help.
Lets say you are doing medicine and you meet the cut-off point.
if the total fees you have to pay per year is 12,000ghc, the government will pay lets say 8000ghc and then you will pay the 4000ghc left. If you are a fee paying student, you will have to pay all the 12,000ghc yourself.

Normally, the part of the fees that the government pays is called tuition whiles the part you pay is called university approved charges.
